Linkage to Care

Care Resource offers client-centered linkage to care services for anyone newly diagnosed with HIV, those previously diagnosed and recently relocated, those seeking to enter our network of health centers or individuals who have temporarily disengaged from HIV care.

Our specially trained staff are dedicated to helping you or your loved one reconnect with life-saving care by leveraging individual strengths and resources.
