Linkage to Care
Our Linkage to Care team helps individuals living with HIV connect to healthcare services, establish care, re-engage after a gap in treatment, or navigate services following a new diagnosis. We provide personalized support to help ensure patients have access to the care and resources they need.
Food Assistance & Nutrition Support
Food insecurity can have a major impact on health. Through Food for Life Network and other support programs, eligible individuals may receive groceries, home-delivered meals, and emergency food assistance. Our team helps connect patients to available nutrition resources that support overall wellness and quality of life.
Housing Assistance
Stable housing is essential to overall health and well-being. Care Resource helps eligible individuals access housing-related support and community resources designed to promote long-term stability. Services may include housing navigation, housing case management, budget planning, job search assistance, emergency rental or mortgage assistance, utility assistance, moving expenses, housing-related deposits, and referrals to specialized housing programs. Our team can also help determine eligibility for long-term housing assistance programs and other available community resources.
Transportation Assistance
Transportation challenges should not prevent access to healthcare and essential services. Through available funding and community partnerships, eligible patients may receive assistance with public transportation to help them remain connected to care and support services.
Emergency Financial Assistance
When available funding permits, eligible individuals may receive support for essential needs such as rent, utilities, and food assistance. Our team can help determine eligibility, explain available programs, and assist with the application process.
Benefits & Prescription Assistance
Understanding available benefits can be complicated. Our navigators and case managers help patients determine eligibility and apply for programs that may reduce healthcare and medication costs. Assistance may include prescription assistance programs, ADAP, AICP, SNAP, and other local, state, and federal programs.
Community Referrals & Resources
When additional support is needed, our team connects patients with trusted community organizations and services that address healthcare, housing, nutrition, financial, and other social needs. We help identify the programs that best fit each individual’s circumstances.
