Before You Submit
To complete your registration, please upload a valid photo ID. You may use your smartphone to take clear photos of your documents and upload them directly with this form.
- If you have health insurance, please upload photos of both the front and back of your insurance card.
- If registering a minor, please upload:
- A copy of the birth certificate, and
- Any guardianship documentation, if applicable.
Please note: Registrations cannot be processed until all required documents have been received.
