Sliding Fee Discount Program

We are committed to making healthcare accessible to everyone, regardless of financial circumstances. You will never be denied care because of an inability to pay. Care Resource accepts most public and private insurance plans.

If you do not have insurance, or your coverage is limited, you may qualify for discounted services if you reside within our service area, which includes Miami-Dade and Broward counties.

Eligibility for the Sliding Fee Discount Program

Eligibility is based on:

  • Household size
  • Household income
  • Current Federal Poverty Level (FPL) Guidelines

Patients with household incomes at or below 200% of the Federal Poverty Level may qualify for discounted services.

Household Size

Household size generally includes individuals who live together and are supported by the same household income.

For purposes of the Sliding Fee Discount Program, household members generally include individuals related by birth, marriage, domestic partnership, or adoption who live together and share financial support.

Household Income

Household income generally includes income received by household members who contribute financially to the household. Depending on your circumstances, household income may include wages and salaries, self-employment income, social security benefits, disability benefits, retirement or pension income, unemployment benefits, child support, alimony. or other sources of financial support

Supporting documentation may be required for each income-earning household member whose income is included when determining eligibility.

Important Information

  • Applications must be submitted within 90 days of receiving services for eligible discounts to be applied.*
  • Sliding Fee Discount eligibility must be renewed annually.*
  • Patients are responsible for providing updated household size and income information each year to maintain their discount status.*

How to Apply or Renew Application

Complete the Sliding Fee Determination worksheet and submit the required documentation in person to the registration desk at any Care Resource location.

Our team will review your household size and income to determine whether you qualify for discounted services.

Required Income Documentation

Please provide proof of income for all household members whose earnings contribute to the family’s total household income. Documentation is required to verify eligibility for the Sliding Fee Discount Program.

One of the following forms of documentation may be accepted:

  • Last two most recent pay stubs showing Gross earnings
  • Letter from employer, shelter, housing program, treatment program or Case Manager
  • Statement from Social Security stating earned income for current year
  • Social Security TPQY (Third Party Query System) form for current year
  • W-2 Forms
  • Proof of unemployment benefits
  • Third Party Letter of Support form (Open PDF fillable form)
  • Self-Declaration of Income form (Open PDF fillable form)

Insurance Information

We welcome patients with insurance. However, if your insurance does not cover a service, or only covers a portion of the cost, you will not pay more out of pocket than you would under the Sliding Fee Discount Program if you qualify.
