We are committed to making healthcare accessible to everyone, no matter your financial situation. You will never be denied care because of an inability to pay.
We accept most public and private insurance plans. If you don’t have insurance, or your coverage is limited, you may qualify for discounted services at Care Resource.
To apply, simply complete a Sliding Fee Determination Worksheet. Our team will review your household size and income to determine if you qualify. You’ll be asked to provide proof of income as part of the process.
One of the following forms may be enough:
- Last two most recent pay stubs showing Gross earnings
- Letter from employer, shelter, housing program, treatment program or Case Manager
- Statement from Social Security stating earned income for current year
- Social Security TPQY (Third Party Query System) form for current year
- W-2 Forms
- Proof of unemployment benefits
- Third Party Letter of Support form
- Self-Declaration of Income form
Payment & Discount Policy
We offer a sliding fee discount based on your income to help make care affordable:
- At or below 100% of the Federal Poverty Level (FPL): You’ll pay a small, reduced fee that does not reflect the full cost of services.
- Between 100% and 200% of FPL: You’ll receive a discounted rate.
- Above 200% of FPL: Standard fees apply with no discount.
If you’re unable to pay your balance in full, flexible payment plans are available.
Insurance Information
We welcome patients with insurance. If your insurance does not cover a service—or only covers part of it—you won’t pay more out of pocket than you would under our sliding fee discount program.
