At Care Resource, we are committed to providing high-quality care in a safe and supportive environment. If you believe your rights have been violated or have concerns about the care or services you received, you may submit a grievance either in person at one of our health centers or by completing the online form below.
If you feel that you have been treated unfairly or that the services you received did not meet your expectations, please follow these steps:
- Notify the Care Resource staff member providing your care about your concerns or issues.
- If a resolution cannot be reached, you may ask to speak with the Department Supervisor or Manager, who will work with you and the staff member to address and resolve the concern.
- If you remain dissatisfied, you may request an appointment with the department’s Director to further review your grievance.
- If your concern is not resolved to your satisfaction within Care Resource, you have the right to seek assistance from an external review body. Contact information for these agencies is provided below.
