To continuously enhance the patient experience, target service improvements and deliver improved patient-centered care, Care Resource has rolled out a real-time exit survey at the medical checkout areas in all of their office locations on electronic tablets. This feedback is valuable not only in helping Care Resource identify and correct issues, but also recognize physicians, nurses, and other staff who receive positive feedback.
From the exit-survey, the health center can gather immediate feedback and quickly hone in on a potential problem. For example, if one doesn’t explain discharge instructions, we can be alerted and follow-up as needed.
The checkout survey asks what service(s) were used. These services include Medical, Dental, and Behavioral Health, Health Promotion, Linkage, Food-for-Life Network, Case Management, Housing (HOPWA), Testing, Project AIDS Care (PAC) or other. Moreover, the survey rates if health center staff acknowledged and greeted you warmly, actively listened to your concerns, knew what we were doing while serving you, anticipated your needs and provided for them and addressed your issues promptly. There is an optional section for comments as well.
