Care Resource offers a broad range of services to respond to the needs of the underserved and vulnerable populations in the counties of Miami-Dade and Broward. These services include general primary care; oral health care; mental health and substance use disorder services; case management; and others.
Access to these services is extremely important for us. We believe that regardless of socioeconomic status, race, gender, religion, immigration status or sexual orientation, everyone should have access to these services. In fact, no patient should be denied services because of their inability to pay.
Care Resource accepts most public, private and commercial insurance plans. However, if you are uninsured or underinsured, you may qualify for a discount when accessing many services at Care Resource.
To qualify for such discount, all patients will need to complete the sliding fee determination worksheet. Our staff will determine your discount based on your household/family composition and income. Individuals interested in applying for the discount must provide proof of household/family income.
One of the following forms may be enough:
- Last two most recent pay stubs showing Gross earnings
- Letter from employer, shelter, housing program, treatment program or Case Manager
- Statement from Social Security stating earned income for current year
- Social Security TPQY (Third Party Query System) form for current year
- W-2 Forms
- Proof of unemployment benefits
- Third Party Letter of Support form
- Self-Declaration of Income form
In general, patients at or below 100% of the federal poverty level (FPL) will pay a nominal fee, which doesn’t reflect the actual cost of the service. Individuals between 100% FPL and 200% FPL will get a discount. Anyone above 200% FPL will receive no discounts. Payment options are available if you can’t pay your charges in full.
Our health centers may serve patients with third party insurance that do not cover, or only partially covers fees for certain health center services. In general, these patients are charged no more for any out-of-pocket costs that they would have paid under the applicable SFDS discount pay class.
