RPM Services

RPM, or Remote Patient Monitoring, uses physiologic monitoring devices to allow your clinician to monitor acute and chronic conditions, supporting you to reach your treatment goals. The program targets a range of conditions including hypertension, diabetes, and individuals with a body mass index (BMI) greater than 30. Care Resource also offers RPM services for clients living with Human Immunodeficiency Virus (HIV) who may benefit from additional support and services to improve viral suppression.
