Remote Patient Monitoring Program: Enhancing Access to Healthcare

Remote Patient Monitoring (RPM) is a proactive approach that enables clinicians to monitor patients’ health conditions from a distance closely. This program targets a range of conditions, including hypertension, diabetes, and individuals with a body mass index (BMI) greater than 30. Care Resource aims to achieve multiple objectives by employing RPM services: reducing hospitalizations whenever possible, enhancing patient engagement in their care, and ultimately fostering positive health outcomes through comprehensive support.

One of the standout features of Care Resource’s RPM program is its accessibility. Enrollment is streamlined through a user-friendly mobile app, allowing patients to choose from various devices such as blood pressure machines, scales, and glucose tracking. Alternatively, patients can opt for a standard kit, which includes a state-of-the-art 5 G-enabled Samsung tablet with Bluetooth functionality, ensuring seamless communication and data transfer.

Once enrolled, patients receive individualized care plans to address their specific diagnoses. These plans incorporate a range of educational resources, including informative videos, symptom survey questions, and medication reminders. Empowering patients to take control of their health serves as a catalyst for long-term positive outcomes.

Remote Patient Monitoring is a valuable tool to assist patients in meeting their health goals. Creating individualized care plans specific to patient needs and maintaining close contact with a member of the Virtual Services Department encourages the formation of an alliance that aims to foster improved health outcomes.
