b'LINKAGE & RETENTIONThe linkage coordinators assist patients that are recentlydiagnosed with HIV or previously diagnosed (relocating,transferring care or lost to care) with the purpose of linkageto medical services. The linkage team uses a strength-based model which helps the patient identify treatment supporting goals, build coping skills as they receive assistance withappointment setup, medical documentation, stable housing, and adherence to treatment.Once the patients first medical appointment is secured, they are introduced to a Case Manager who will continue to provide care coordination and retention support. Linkage Assistants can also provide home visits to locate patients that are not showing up to their scheduled medical appointments. A signed consent and/or a certified referral might be required at the time of enrollment for a patient to receive home visits.-47-'
